The WelbeHealth PACE program helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. It's our mission to serve the most vulnerable seniors with better quality and compassion in a value-based model, and the Registered Nurse (RN) Case Manager directly guides this care as a primary voice on the interdisciplinary team (IDT).
Reporting to the Clinic Manager, the RN Case Manager delivers and oversees all aspects of participant care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.
Essential Job Duties:

<li>Conduct clinic nursing assessments to determine the nursing care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans</li>
<li>Deliver and document nursing interventions as agreed upon in the participants' care plans including but not limited to maintaining a healthy and safe environment, promptly and accurately responding to physician orders, and correctly administering medications</li>
<li>Work with the WelbeHealth PACE Physician and other members of the IDT to coordinate participant care needs</li>
<li>Develop a plan of care based on the nursing process, which incorporates the plans of other disciplines and continuing care needs, and includes the patient and family in developing or revising the plan of care</li>
<li>Instruct the participant, family, and caregivers regarding the disease process, self-care techniques, and prevention strategies </li>

Job Requirements:

<li>Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred</li>
<li>Unencumbered California (RN) License</li>
<li>Current Basic Life Support (BLS)/First Aide Certification required</li>

Physician - OB/GYN Requirements:&nbsp;
EDUCATION AND EXPERIENCE
<ul>
<li>Graduate of an accredited medical school with a valid California medical license.</li>
<li>Current DEA certificate and CPR certification required.</li>
<li>Extensive knowledge and experience in clinical practice.</li>
<li>Ability to supervise Nurse Practitioners or Physician Assistants&nbsp;</li>
<li>Excellent problem-solving, communication, and critical thinking abilities.</li>
<li>Ability to perform under pressure and manage urgent clinical situations with sound judgment.</li>
<li>Bilingual skills and knowledge of community health needs are a plus.</li>
<li>Must be physically able to stand for long periods and respond effectively in clinical emergencies.</li>
<li>Interaction with patients, clinical staff, and the general public in a fast-paced healthcare setting.</li>
<li>Must comply with HIPAA and Corporate Compliance Program, including confidentiality, integrity, and ethical behavior standards.</li>

Physician - OB/GYN Essential Functions:
<ul>
<li>The Obstetrician/Gynecologist reports to the Chief Medical Officer and is responsible for providing high-quality healthcare to female patients, focusing on reproductive health.</li>
<li>This role includes monitoring patients throughout prenatal, natal, and postnatal stages; performing necessary tests; prescribing treatments; and delivering babies using the safest and most appropriate methods.</li>
<li>Additionally, the OB/GYN collaborates with nurse practitioners and physician assistants to develop, implement, and review clinic policies and services, especially those related to federally funded programs.</li>
<li>Average volume of patients: roughly 7–8 per day,150 per month. or 3-5 deliveries per 24hrs of call.</li>
<li>Robotics Privileges available</li>
<li>Volume of deliveries: Program has own MFM to assist with high risk and residents and fellows to assist with rounding.</li>
