<li>Serve as the primary nurse for a caseload of hospice patients, providing skilled nursing care guided by the plan of care.</li>

<li>Coordinate services across the care team—including physicians, social workers, chaplains, aides, volunteers, and contracted providers (PT, OT, Speech, Dietary, Pharmacy).</li>

<li>Complete all clinical documentation accurately and on time.</li>

<li>Participate in the on-call rotation, including some weekends and holidays.</li>

The Community Manager I is responsible for:

 Be the clinical anchor for patients facing serious illness. As the Palliative RN Case Manager, you lead comprehensive, person-centered care—identifying physical, psychological, and social needs; coordinating the right interventions; and keeping patients and families informed and supported throughout the journey. Job Description
The Resident Services Supervisor reports to the Community Manager and is responsible for the overall maintenance of the physical site and supervision of entire Resident Services team.
RESPONSIBILITIES
<ul>
<li>Work closely with Community Manager to maintain integrity of property within established budget.</li>
<li>Over site of all maintenance projects.</li>
<li>Supervise and assist in all maintenance issues including, but not limited to, electrical repairs, plumbing repairs, carpentry work, custodial work and concrete, masonry, roofing fencing and signage when needed.</li>
<li>Perform touch-up painting, interior and exterior, as needed.</li>
<li>Perform carpentry work, such as fitting doors, freeing windows, replacing and building shelves, sanding and refinishing floors.</li>
<li>Train subordinate employees.</li>
<li>Monitor all incoming after-hour emergency calls and participate as needed.</li>
<li>Be responsible for inventory control and utilization of maintenance materials.</li>
<li>Keep storage/maintenance shop facilities and equipment clean and in orderly working condition.</li>
<li>Schedule subordinate maintenance personnel and work order requests, adhering to maintenance priorities.Ensure that work orders were completed within 24 hours, unless otherwise stipulated, and all needed follow-up or scheduling is accomplished in a timely manner.</li>
<li>Assist in scheduling and completing apartment inspections, including annual and turnover inspections.</li>
<li>Be responsible for the refurbishing of apartments prior to resident occupancy.</li>
<li>Keep accurate records regarding preventive maintenance, work orders, apartment refurbishing, annual inspections, inventories, purchase orders.</li>
<li>Perform scheduled maintenance on all equipment based on the manufacturers' recommendations and operating manuals.</li>
<li>Periodically inspect all units, buildings, and common areas, performing repairs and janitorial assistance as needed.</li>
<li>Have knowledge regarding water and gas meter shutoffs, all apartment and fixture shutoffs, sewer cleanouts and post map of the same.</li>
<li>Make sure storage areas and entrances are locked and adequate lighting in those areas is maintained as well as throughout common areas of property.</li>
<li>Have constant knowledge regarding contracts and vendors, their services and goods.</li>
<li>Negotiate with vendors to ensure best pricing on services and goods.</li>
<li>Responsible for Performance Reviews and disciplinary action process for subordinate employees.</li>
<li>Responsible for execution and record keeping of Preventive Maintenance Program.</li>
<li>Must be conscious of a safe work environment.</li>
<li>Assist in Resident Services Training programs.</li>
<li>Must have current license and be insured to drive for company business.</li>
<li>Must have home telephone and inform supervisor of current phone number.</li>
<li>Work with the Capital Improvement Manager to plan and implement all capital improvement projects.</li>
<li>Ensure compliance with all MSDS books, labeling of bottles and other materials.</li>
<li>Must attend Company sponsored events that relate to the development of the team, which, from time to time, may include overnight stays at locations away from the employee's home.</li>
<li>Other duties as may be needed or required at the request of the Community Manager or other supervisors.</li>

Associate Behavioral Health Specialist
The WelbeHealth PACE program helps seniors stay in their homes and communities by providing medical care and community-based services. We like to do things a little differently, going the extra mile and making a difference in our participants' lives.
The Associate Behavioral Health Specialist (ABHS) is accountable for providing behavioral health counseling services under an LCSW level or equivalent clinician in accordance with supervisory requirements from the Licensure Board. The ABHS will work with participants identified by the interdisciplinary team (IDT) to deliver behavioral health assessments, counseling, crisis interventions, and other therapeutic interventions. The ABHS will work independently with clinical supervision and oversight, and will be eligible to accrue clinical hours required for Licensed Clinical Social Worker (LCSW) licensure. The ABHS will collaborate with the PACE physician, social worker, consulting psychiatrist, or psychiatric mental health nurse practitioner (PMHNP), and other IDT members as necessary.
Essential Job Duties:

<li>Conduct behavioral health assessments and treatment planning under the guidance of an LCSW level or equivalent clinician, in accordance with supervisory requirements from the Licensure Board, to determine participants' needs, preferences, and goals, and to develop and implement tailored treatment plans</li>

Job Requirements:
