What You’ll Do

- Coordinate services end-to-end: initiate timely referrals (e.g., PT, OT, MSW, ST, Dietitian, DME, and specialized programs) and report needs to clinical leadership and/or practitioners.
- Monitor clinical changes: re-evaluate status, consult with clinical leadership/practitioners, and ensure risk adjustment (HCC) and CMS 5-Star measures are addressed when applicable.
- Educate patients and families: teach diagnosis-specific self-management in clear, practical terms.
