Pharmacist-Infectious Disease

Job Description

Job Objective:

• Applies specialized knowledge and skills in the purchasing, storing, prescribing, assessing, preparing, dispensing and monitoring of pharmaceutical agents and investigational drugs used in the treatment of acute care in collaboration with the multidisciplinary healthcare team for the purpose of optimizing healthcare outcomes. Manages the Antimicrobial and Diagnostic Stewardship Program (ADSP) in collaboration with the medical staff, including Infectious Disease physicians and other health professionals.

Qualifications:

Education:

• Required: Pharmacy degree from an accredited School of Pharmacy
• Preferred: Completion of an ASHP-accredited pharmacy residency

Licensure/Certification:

• Required: California Board of Pharmacy License; Board of Pharmacy Specialties Certification in Infectious Diseases or Society of Infectious Diseases Pharmacists (SIDP) Antimicrobial Stewardship Certificate for Acute Care within 1 year of hire
• Preferred: ASHP and/or CSHP membership; Board Certification in Pharmacotherapy

Experience:

• Required: Two (2) years PGY2 residency training in Infectious Disease OR PGY1 residency training and two (2) years of experience in Infectious Disease (three (3) years of experience) OR four (4) years of experience in Infectious Disease OR Fellowship training in Infectious Disease
• Preferred: Acute-care hospital experience as a Clinical Pharmacist or equivalent experience
