POSITION DESCRIPTION: School Site Therapists The therapists will work directly with the Executive Director, their Clinical Director and the School-Based Program Coordinator. They will provide clinical evaluations, assessments, treatment, and consultation as well as educational services when necessary. As a member of a clinical team, he/she will work in tandem with school personnel when appropriate to improve the behavioral and emotional well-being of the students they are serving. Therapists will provide services in schools, and rural communities, and in any other organizational environment of the Coachella Valley Unified School District system. They may design and deliver educational programs for community constituencies as well.
Minimum Job Qualifications: Personnel must have a Master’s or higher level of education, and be registered as an Associate or licensed with the California Board of Behavioral Sciences as a Marriage and Family Therapist, Professional Clinical Counselor, or Clinical Social Worker. Master’s level professional Practicum Student (i.e. Trainees as an MFT, PCC, or CSW) are under the supervision of an LCSW. The Latino Commission Counseling Center retains the discretion to add or to change duties of the position at any time.
Job Duties and Requirements:
• Provide social and emotional support to students both individually and in groups. • Provide classroom-based counseling and make clinical observations of students in the classroom.
• Provide therapeutic treatment and support for individuals and groups (i.e. students and their families.)
• Maintain clinical files for each student which includes but not limited to attendance records for individual and group sessions as well as session notes, child abuse reports, records of significant contacts etc.
• Attend meetings pertaining to student needs (i.e. SST, SART, IEP, reintegration meetings for suspensions, expulsions, 5150’s etc.)
• Consult with other staff as needed regarding clinical issues affecting the students being served.
• Attend and participate in monthly clinical staff meetings or as scheduled.
• Assist in the development and implementation of the campus psychosocial curriculum and parent training projects.
• Develop individual treatment goals and plans.
• Be able to work within a multidisciplinary team.
• Apply therapeutic theory, techniques and practice specific to the targeted population.
• Be culturally informed and sensitive to the population they are serving.
• Be flexible in order to adapt to the frequently changing demands of a school environment.
• Follow all classroom/school policies and procedures thereby setting an example to students.

<li>Responsible for utilizing brief behavioral health therapeutic interventions as necessary to improve the members ability to manage their own health</li>
<li>Responsible for brief crisis interventions and warm hand-offs to local crisis resources as needed</li>
<li>Educate members and families about chronic medical and behavioral health conditions to improve health literacy and assists in health promotion</li>
<li>Assist with the coordination of medical and behavioral health access issues with PCP offices, specialists, and ancillary services</li>
<li>Advocate for members to receive the highest quality of care, in a timely manner, within IEHPs network</li>
<li>Provide effective care management for assigned members, including care coordination, linkage to resources, support in transitions of care, facilitating member self-efficacy and self-management, and all other activities associated with high quality, evidenced-based care management</li>
<li>Engage and collaborate with staff outside the BH department, or with those outside BH discipline, to provide integrated care to assigned members</li>
<li>Responsible for assisting members with care coordination needs</li>
<li>Conduct comprehensive, holistic assessment according to the scope of clinician-both telephonically as well as in person</li>
<li>Aid RN CM in overall review of member Community Health Assessment (CHA) upon completion by another care team member</li>
<li>Works with members to identify health/wellness goals and incorporates these goals into the members shared care plan that facilitate communication among members and providers</li>
<li>Participate in inter/transdisciplinary care team meetings to share information, update, and inform care plan</li>
<li>Coordinate with internal and external health partners to support member comprehensive care needs</li>
<li>Responsible for ongoing engagement with members, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as Motivational Interviewing) that promotes collaboration between the member and his or her medical/behavioral team, as well as to increase the members sense of control over their whole health</li>
<li>Engage in proactive, member-centered utilization and quality review of Behavioral Health and Care Management services by members</li>
<li>Understand and abide by all departmental and companywide policies and procedures</li>

Responsibilities
Matrix Provider – What to Expect:
