Responsibilities
About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:

<li>Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Administer and order point of care and lab-based testing as needed</li>
<li>Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary</li>
<li>Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities</li>

Qualifications
Skills &amp; Experience That Shine

<li>Master’s Degree or commensurate experience and satisfactory completion of NP licensure</li>
<li>Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed</li>
<li>AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty</li>

Job Duties

<li>Evaluate and manage chronic pain conditions as part of comprehensive care plans, integrating pharmacologic and non-pharmacologic therapies.</li>
<li>Collaborate with pain management specialists and complementary providers (chiropractors, acupuncturists, massage therapists) to develop individualized multimodal pain treatment strategies.</li>
<li>Perform interventional pain procedures as trained and credentialed, including joint injections, trigger point injections, and other minimally invasive therapies.</li>
<li>Educate patients on safe pain management practices including opioid stewardship, alternative therapies, and self-management techniques.</li>

 Job Title Registered Nurse- Home Health Position Registered Nurse Job Description Summary Gives Report To: Director of Patient Care Services/Director of Nursing/Nursing Supervisor The registered nurse plans, organizes, and directs home care services to improve quality life for the patient. Roles &amp; Responsibilities

<li>Completes an initial, comprehensive assessment of patient to determine home care needs</li>
<li>Creates a plan of care which establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions</li>
<li>Provide ongoing reassessments to regularly revise patients plan of care as patients status and needs change</li>
<li>Use health assessment data to determine nursing diagnosis</li>
<li>Reports all changes in patient condition to clinical supervisor</li>
<li>Provides comprehensive assessment and nursing care based on nursing process</li>
<li>Determines and documents nursing diagnosis and interventions to meet patient needs</li>

<li>Participates in QAPI programs and initiatives</li>
<li>Participates in professional development and skills training as required or needed</li>
<li>Attends monthly in-services</li>
<li>Follows all agency policies and procedures, including in Infection Control, Emergency management, medication management, emergency management, pain management and overall patient care policies and procedures</li>
<li>Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies</li>
</ul>
Qualifications 
<ul>
<li>Licensed Registered Nurse authorized to practice professional nursing in the state</li>
<li>Holds and maintains current CPR certification</li>
<li>Must have a valid and active drivers license</li>
<li>Must have car insurance</li>
<li>Liability insurance is optional</li>
<li>Must have an automobile in good working condition</li>
<li>Must have at least one year of experience as a professional nurse, preferably in home health care, hospice or public health</li>
<li>Bachelor's degree in nursing is optional</li>
<li>Must have great interpersonal and communication skills</li>
<li>Must be proficient in the use of computers for computerized clinical documentation</li>
<li>Must be capable of autonomous work with basic safety knowledge</li>
<li>Graduate of an accredited school of nursing</li>
